Emergency nurses know what it’s like to suffer pangs of moral distress

A recent study published in the Journal of Clinical Nursing reveals that emergency room nurses experience moral distress at rates and intensities comparable to the physicians they work beside. The research, which surveyed hundreds of ER nurses across multiple hospital systems, found that the distress stems from a complex web of systemic pressures rather than isolated incidents.

Moral distress occurs when a healthcare professional knows the ethically correct action to take but is prevented from taking it by institutional constraints. For ER nurses, these constraints often include overcrowded waiting rooms, insufficient staffing, and pressure to discharge patients before they are stable. The study highlights that nurses frequently face situations where they must ration care, prioritize patients based on administrative metrics, or follow orders they believe are not in a patient's best interest.

The consequences are severe. Many nurses reported symptoms of burnout, anxiety, and a growing sense of cynicism toward their work. Some described feeling trapped between their duty to patients and the demands of a profit-driven healthcare system. The study also notes that moral distress is not a uniform experience. It varies by shift, patient volume, and the level of support from hospital leadership.

Researchers call for systemic changes, including better staffing ratios, more robust ethics consultations, and a workplace culture that allows nurses to voice concerns without fear of retaliation. Without such changes, the study warns, the exodus of experienced ER nurses will likely continue, further straining an already fragile emergency care system.
