More Medicaid coverage hasn’t curbed increased firearm suicides among most men

A new analysis of national data shows that while broader Medicaid access has helped some groups, it has not reversed a troubling upward trend in firearm suicides among most adult men. The study, published in a leading health policy journal, found that expanded Medicaid coverage was linked to a meaningful decline in suicide rates for only one demographic: white men aged 35 to 49.

For all other groups of men, including younger adults, older men, and men of color, the rate of firearm suicide continued to climb even after states expanded Medicaid under the Affordable Care Act. Researchers examined data from 2000 to 2020, comparing states that expanded the program with those that did not. They focused on firearm suicides because they account for the majority of gun deaths in the United States and are often linked to untreated mental health crises.

The findings suggest that simply having health insurance may not be enough to address the root causes of suicide, such as depression, substance use, and access to lethal means. The authors note that white men ages 35-49 may have benefited because they are more likely to use outpatient mental health services once they gain coverage. Other groups, however, may face barriers like stigma, lack of nearby providers, or a preference for informal support.

The study's lead author said the results do not mean Medicaid expansion is useless, but rather that it must be paired with stronger suicide prevention efforts, such as crisis hotlines, firearm safety counseling, and community-based programs. Without those, insurance alone will not save lives. The research adds to a growing body of evidence that health coverage is only one piece of a much larger puzzle in reducing gun suicides.
